DO NOT MAIL REGISTRATION FORMS AFTER AUGUST 1
In order to avoid "Check is in the Mail" situations, please do NOT mail registration forms and/or payments after August 1, 2012. Instead, please bring your form and payment (cash) on event day and register at the check-in table. If you mail your payment and we have not received it, you will need to pay at the door and we will reimburse you upon receipt of your mailed payment. Remember: CASH ONLY on event day. Mahalo!

1. Review the cost and payment information below. (Note: Late registration fee is now $70.)
2. Click the link below to download and print the HAWAII SIX-0 Registration Form. (Note: Clicking this link will open a PDF file. If you wish to return to this website, click the BACK arrow in your browser.)

3. Complete the form and mail it in with your payment ($70 AFTER JULY 21, 2012).
Please include an email address (if you have one). Your Registration Confirmation will be sent to you via email. To avoid problems at the door, make sure you "receive your Confirmation and are on the Attendee List on the Who's Comin' page of this website."
4. From now to August 11, 2012, count down the days to HAWAII SIX-0 !!!
